Is hard water bad for your health? What the evidence says

Drinking hard water does not harm your health: hardness does not even appear among the parameters with a health-based limit value. We go through what the reference bodies say about the three most common worries, and point out the one thing that really does deserve attention, which is not where most people look for it.

No. Drinking hard water is not harmful to a healthy person. Limescale in water is calcium carbonate, that is, dissolved calcium and magnesium, and neither poses a risk at the concentrations found in drinking water. Hardness does not appear among the parameters with a health-based limit value, either in Spanish regulations or in the European drinking water directive, precisely because no adverse effect is attributed to it.

The World Health Organization has reviewed the question several times and has not set a health-based guideline value for hardness. Its position is the opposite of the one usually circulated: the calcium and magnesium in water contribute to daily intake of two minerals that a good part of the population does not fully cover through diet.

Why hardness is not regulated as a risk

Drinking water parameters fall into two groups. There are those whose limit is justified on health grounds, such as lead, nitrates or arsenic. And there are indicators, monitored because they affect taste, appearance or how the water behaves in the installation, but not the person drinking it. Hardness belongs to the second group, and in practice it is not even assigned a maximum value.

Put another way: water at 55 °f is as drinkable as water at 8 °f. What differs between them is not safety but the service life of a cylinder and the state of a shower screen. If you are unsure what hardness you have at home, Water hardness in Spain explains how to look it up and how to measure it.

Calcium and magnesium: how much water actually contributes

Water at 35 °f contains roughly 350 mg of calcium carbonate per litre, which amounts to an appreciable contribution of calcium and magnesium for anyone drinking a litre and a half to two litres a day. It does not replace diet, but it is not negligible either: in hard water areas it can cover a significant fraction of the recommended daily magnesium intake.

That matters when considering softening a whole house, because removing calcium from drinking water brings no health benefit and removes that contribution.

The three most repeated worries, one by one

Does hard water cause kidney stones?

This is the most widespread belief and the evidence does not support it. Calcium oxalate stones, the most common kind, are associated with urine volume, with oxalate, salt and animal protein intake, and with individual predisposing factors. The WHO review finds no sufficient basis for attributing kidney stones to water hardness.

In fact, the usual clinical advice for anyone who has had stones is to drink more water, with no restriction on its hardness, because the dominant protective factor is dilution. Restricting dietary calcium is generally counterproductive.

Does limescale build up in your arteries?

No. The image of a scaled-up pipe transfers intuitively to the human body, but the two processes have nothing in common. Arterial calcification is a complex biological phenomenon tied to inflammation of the vessel wall and to calcium metabolism, not to carbonate that enters through the mouth. Calcium from water is absorbed in the gut and metabolised like calcium from any food.

Interestingly, epidemiological studies exploring the relationship between water hardness and cardiovascular mortality point the other way, with a possible inverse association. The bodies that compile them consider the evidence insufficient to establish causality, so the prudent conclusion is simply that hard water does not harm the cardiovascular system.

Does hard water dry out skin and ruin hair?

Here there is something real to explain, though it has nothing to do with drinking it. In hard water, soap reacts with calcium and magnesium to form an insoluble residue that deposits on skin and hair. That film is what produces the tight feeling and the rough hair, and it explains why people in hard water areas tend to use more product than they need.

There is also research exploring a link between domestic water hardness and atopic dermatitis in early childhood. The results are suggestive and worth following, but they do not currently allow a causal claim. For sensitive skin, the practical and uncontroversial advice is to use less soap and rinse thoroughly, rather than to change the water.

The point that does deserve attention: sodium in softened water

The interesting question is not whether you can drink hard water, but whether you can drink salt-softened water. And there the answer needs a caveat, because a resin softener does not remove calcium: it exchanges it for sodium.

The conversion is direct: for every French degree of hardness removed, roughly 4.6 mg/L of sodium is released. That rule lets you work out the result for any installation.

The parametric value for sodium in drinking water is 200 mg/L. It is an organoleptic indicator rather than a toxicological limit, but it marks the boundary of what is acceptable.

This is why a properly set softener never runs at 100 %: it is adjusted with a blend that leaves residual hardness, so that final sodium stays low. And it is why the classic advice from serious installers is to leave the kitchen tap unsoftened, or at least the drinking water point.

What physical treatment changes and what it does not

Physical treatment frames the question differently. It does not remove calcium, does not add sodium and does not introduce anything into the water: it acts on how the carbonate crystallises so that it stops bonding to surfaces. The practical result is that the water leaving the tap has exactly the composition of the water that entered.

  • The minerals stay: calcium and magnesium remain in the water and still count towards intake.
  • No sodium or any other ion is added.
  • The pH is unchanged.
  • The taste does not change, because the mineral content does not change.
  • No consumables, no regeneration and no discharge to drain.

Put precisely, and without overstating what the unit does: physical treatment does not improve the water's health quality, because there was nothing to improve. It solves an installation problem without touching what you drink.

What if I want to change the taste of the water?

That is a different need and it deserves a different solution. A chlorine taste is corrected with an activated carbon filter at the point of use. Deep demineralisation is achieved with reverse osmosis, which removes most dissolved salts, including the minerals with nutritional value, and which produces a reject stream worth accounting for.

Neither protects the installation from scaling, because both are fitted at the point of use rather than at the main inlet. They complement limescale treatment; they do not replace it.

Frequently asked questions

Can you drink softened water?

Yes, provided the resulting sodium stays within reasonable values, which is achieved by setting the unit to leave residual hardness. It is not advised for making up infant formula or on a strict low-sodium diet. Good practice is to leave the kitchen tap unsoftened.

Does physical treatment change the taste of water?

No. Taste depends on mineral content and chlorine level, and physical treatment alters neither. The composition of the water leaving the unit is the same as the water entering it.

Does boiling water remove limescale so it is better to drink?

Boiling precipitates part of the carbonate, which settles at the bottom of the vessel. It slightly reduces dissolved calcium, but brings no health benefit because that calcium was never a problem. The reason to boil water is microbiological, not about hardness.

Does hard water damage clothes?

Yes, it plays a part, though indirectly. Calcium reacts with detergent and reduces its effectiveness, and the residues fix in the fibres, which lose softness and colour over successive washes. It is an effect on the fabric and on detergent use, not on health.

Can limescale carry bacteria?

Limescale itself is not a biological contaminant, but a scaled surface is rough and porous, and that favours biofilm attachment in hot water circuits. It is a hygiene argument for keeping installations free of deposit, not a risk of the water you drink.
